About the Position:
Los Angeles County’s Homeless Initiative Strategy D2 – Expand Jail In Reach, provides Release Planning and Case Management Services to individuals in Los Angeles County jail facilities (Jails) who are at risk of homelessness at release, through in-reach by community-based organizations that initiate services in custody and continue to provide case management in the community after release (D2 Program). The D2 Program is coordinated by the Los Angeles County (County) Sheriff’s Department (LASD) and Los Angeles County Department of Health Services (DHS). The participants served by the Contractor shall be individuals that are currently incarcerated in Jails and/or is transitioning back into their community.

What You Will Do:
The Case Manager will work to provide case management services for a variety of students with complex health, functional, and social needs. Responsibilities include:
- Work to decrease homelessness and future justice involvement by providing release planning to individuals in custody in the jails, and case management as they transition back into the community
- Conduct regular visits to jails and keep County and LASD informed of its case manager schedules
- Collaborate with partners from LASD and DHS to engage and assess individuals currently incarcerated in the jails
- Establish regularly scheduled case conferences with County staff to discuss the individuals’ cases
- Maintain a caseload at or close to the maximum of thirty (30) participants
- Ensure students are participating in grant goals
- Maintain concise and comprehensive case records on all participants seeking services
- Collaborate with other area agencies and partners involved with the participant to ensure a coordinated effort to best serve the participant
- Provide transportation assistance to housing and appointments by means of company/personal vehicle, bus fare/pass or private vendor.
- Will be required to visit county jails
